Multiple joint pains and SLE

 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. 


Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. 




30 yr old female came to the casually with chief complaints of

Worsening of shortness of breath from 2days 

FEVER, continuous, not resolving on medications (pyrexia of unknown origin). 

-MALAR RASH on dorsum of nose and cheeks


Hopi:

Patient was apparently asymptomatic 6 years back and then developed generalised body aches 

Then multiple joints were involved, mainly being knee and elbow 

There was also associated hairloss 

She visited multiple hospitals,and the previous diagnosis was diagnosed with autoimmune disorder 

Association of pedal edema 

Then 2 months back observed episodes of shortness of breath 

2days back the symptoms worsened 





 General examination:

Vitals: 

Temperature:37.8

RR:20bom

Bp: 110/80 

Pallor present 

Icterus abs 

Cyanosis absent 

Lymphadenopathy absent 

Flat nails

Non scaring aloppecia ,hyperpigmentation dicoid rashes in face 



Black discolouration of oral mucosa 



Systemic examination:

Abdominal examination:

Soft and non tender,mildly distended, inverted umbilicus 

CNS : no focal neurological deficits 

CVS:

S2heard,ni murmurs heard 

Labs:

Normocytic normochromic anemia with leukocytosis 



Diagnosis:

Acute cardiogenic pulmonary edema 

And positive SLE
